Metabase Pulses send to Slack & email taking to wrong URL

We have Pulses that are sent to Slack & Email every day. In recent days, we have experienced that those pulse is taking to wrong link eg - http://11.11.111.111:1111/question/0000. Does anyone also have such experience and knows what causes it? And how can it be fixed?
Any leads would be helpful

Yes, someone has probably accidentally change the Site URL in Admin > Settings > General.
